Situation in germany: calls to +354-380 work fine from T-Com and Arcor landlines. No known call-by-call-Provider connects (like 10-10 numbers in the US). All cellphone providers connect - but they charge a fortune for international calls anyway so they dont care. VoIP works but very patchy (Sipgate.de, simply-connect.de).
