I'm headed to the US and I unfortunately have a locked iPhone that both apple and my provider are telling me they cannot unlock. The unlocking places are telling me they can't do it either because I upgraded to iOS5.

I've decided that I would buy a T-Mobile 4G Mobile HotSpot and get the prepaid month pass. To buy this device in the US, do I need to provide a passport or US address? Or Something? Any other advice? Sims: Fido (expired), GlobalSim, UM |

The only time I purchased a T-Mobile prepaid all I had to do was pay for it. When I activated it online they asked for my address and contact information. That was some time back.

T-Mobile doesn't ask for identification. You should have no problem registering online but may have to pound a little bit to get your address to work. Remember that T-Mobile's 3G is mostly on the 1700 MHz band which is not used in many places. They have a little bit on the 1900 MHz band. If you have a hotspot for Wind it will work just fine on TMobile.
