dict.cc | Englisch-Wrterbuch | German Dictionary and Google Translate may help you filling out the form. For forum veterans like you I also might find a solution for shipping the SIM to the US. Just send me a PM if you need a solomo SIM forwarded to the US.

You're probably better off with Fonic, then, as you can buy it "off-the-shelf" at several ubiquitous German chain stores. The only real disadvantage of it compared to Solomo is it has 60/60 billing, while Solomo has 60/1, but then again it also has data bundles, which Solomo doesn't (they both offer a standard 24 cent/MB data rate). Plus, it has those amusing TV commercials with Bruce Darnell ("Teurrerruhn Hendyvurtrag kundigen", etc.), which I'd imagine can be seen on YouTube.

I would rather go for the VoIP-feature of solomo pro including all those forwarding and callthrough possibilities emerging from the low internal tariff rather than subsidizing fees for a TV appearance.

Hmm, I don't think I'd call him that to his face, he's a former career soldier... but yes, I'd rather support a smaller operation like Vistream that doesn't splash out on big celebrity-driven ad campaigns.
