I dont undertsand the list. It is showing termination fees for US DID to whatever destination in the world the call is routed?

Yep! That's how I see it. That's is the cost to forward your DID to your chosen destination. They only sell US DID/800 number and Canadian DID.

If the VoIP account has forwarding itself, then you could forward a free IPKall or Freedigits/Sipnumber DID straight to the SIP address, without needing Mysipswitch Pbxes Voxalot or similar. And I suppose that if you want a local US code rather than Iowa or Washington state, then Google Voice (ex-Grand Central) might come in handy

True. In my case the most suitable carrier for MX mobile does't have forwarding. Some of the voip forwards can be a bit slow (voipfone.co.uk for example takes ages).

Google Voice to ipkall to mysipswitch does work OK.

When Google Voice offers CF to intl. numbers, that would be even a more simple solution than VoiceStick. I'll more than use up the $1 credit they put in everyone's account when that happens.

Make use of T-M's UMA/wifi free calling from any place in the world with access to wifi. I use an LG G6, wife an S7)
A/o Oct 20, 2013 no need for intl prepaid as T-Mobile U.S. includes voice roaming at 20¢/min (in and out)., unlimited text (in and out), and unlimited data in 140+ countries.

My Plan -[6 lines] U.S. T-Mobile unlimited minutes (incoming and outgoing), unlimited text, fast data on each line. that $145/mo. total! . (In U.S. no surcharge for calling a cell.) If a line exceeds 2G of data in a month, pay $10 more for that line. [That only happens a couple times/year.

You can forward Google Voice to Gizmo Project and buy international call forwarding minutes from Gizmo.
