AFAIR there's one or two Manx SIMs with free incoming in Russia.
In fact it's a problem to find international SIMs with free incoming calls in Russia and Ukraine.

Edit: just found it
http://www.simazing.com/coverage.htm
Free incoming calls in all covered European countries (expect Romania).

Do you think it really matters? What would be a difference between a CPP operator with 10 cents/min termination fee and a RPP one with 5 cents/min termination and 5 cents/min incoming rate?
Moreover, consider that some Manx cards removed Mexico from free-incoming countries lists right after this country changed its payment rule to CPP

What do the abbreviations CPP and RPP stand for?
And what are the consequences for the operators?

CPP - Calling Party Pays. For example, countries with free incoming calls
RPP - Receiving Party Pays. For example, USA/Canada

Not sure about your second question.

Good question. The same for Ukraine: some Manx SIMs (AFAIR Geosim, globalsim.de and "old" Callblue, perhaps some more) used to have free incoming in Ukraine but it changed
