Yep, throwing away the documentation wasn't real bright. 0000 is the default PIN on some Vodafone Italy SIMs. Give it a try - once. If you enter the wrong PIN three times, it won't work, and you need to get the PUK (unblocking code), which was also listed on your documentation.

You can also call Vodafone or send them an e-mail via their website (vodafone.it or 190.it) requesting the default PIN or PUK code. They'll probably give it to you.

If all works out in the end, be sure to set up a web account on the above site. That way you will easily be able to top up your SIM with a credit card (It only takes 3 euros every 10 months to keep it alive, which is probably worth doing if you're planning to come back to Italy in a couple of years).
