Wind data card works but phone card does not

I just purchased a data card and a phone card for Wind. The data card is working well but the phone card appears to connect but when I dial the phone disconnects the call. I tried the sim card in my smart phone and it requests the PIN which I enter and the the confirmation code which I assume is the PUK code on the card but the number is not accepted. Any suggestions?
