Question about buying Mobile Hot Spot in the US

I'm headed to the US and I unfortunately have a locked iPhone that both apple and my provider are telling me they cannot unlock. The unlocking places are telling me they can't do it either because I upgraded to iOS5.

I've decided that I would buy a T-Mobile 4G Mobile HotSpot and get the prepaid month pass.

To buy this device in the US, do I need to provide a passport or US address? Or Something?

Any other advice?
