I've seen this in the past with their core network. The only way I could get reliable service in Chile and the Dominican was to lock the IMSI through subscription codes. This being said Piranha may not have access to these codes or may choose not to release them to end-users.

Long term Piranha need to pressure their supplier to overhaul the STK from automatic only to a Toggle Mobile style IMSI selector. Another reliable approach is that used by eKit on their multi IMSI cards - prefixed PIN numbers - 1+PIN is IMSI 1, 2+PIN is IMSI 2, 3+PIN is IMSI 3 etc.
