First of all: there's no voice or data sim. All Wind sim cards are voice+data simcards.
A new card NEVER asks for the PUK or any comfirmation code. Just the PIN. Then it simply connects to the network (I Wind) and you can call/receive.
I fear the phone you tried the sim could be locked...
