I've used Voxalot for this is the past, but they now charge an annual fee.

More recently I've begun using mysipswitch again. Get an ipkall.com number, point it to the mysipswitch SIP account, and then pick any voip carrier(s) that you have an account on to forward it on (can vary the carrier by destination). I'm using it to send a Washington number to a Mexican mobile.

Neither Voxalot or Mysipswitch are easy to set up, although the new dial plan wizard with Mysipswitch is OK. Both are too flexible, too many options, to be a real consumer solution.
